What hurts the most during pregnancy and what does it feel like?

I am extremely terrified of giving birth. Not even pregnant. But I am a worrier. One thing I wonder about particularly is the umbilical cord.. do you feel it being cut? And is there anyway to go through a WHOLE pregnancy and child birth without any needles? Any other things that hurt?
