How do you know when u have an ectopic pregnancy..i had a death in the fam due to this but nobody even knew ?

she was pregnant.. could we have prevented this..? what signs could we have looked for...?.. or what signs could she have been going through. .. she bled to death.,. and that side of the fam is Jeohva's witness so transfusion was out of the question.
